Our Discharge & Transition Planning Support Services help residents prepare for life after residential treatment by building personalized, practical plans that promote ongoing recovery, independence, and community integration.

What We Offer:

1. Early & Ongoing Discharge Planning
Discharge planning begins at admission and is reviewed regularly to ensure readiness and alignment with each resident’s recovery goals.

2. Individualized Transition Plans
Each resident receives a detailed plan that includes housing, follow-up care, medication support, crisis planning, and ongoing community resources.

3. Coordination With Support Teams
We work closely with outpatient providers, case managers, guardians, and natural supports to ensure smooth, well-communicated transitions.

4. Life Skills for Independent Living
Residents receive coaching in self-care, budgeting, scheduling, and other skills necessary for successful re-entry into the community.

5. Connection to Aftercare Services
We provide referrals and warm hand-offs to outpatient therapy, vocational programs, peer support, MAT services, or other needed care.

6. Follow-Up & Safety Support
Residents leave with a clear plan for continued wellness, including crisis contacts, relapse prevention strategies, and tools to maintain progress.
