Our Coordination of Care service ensures that residents receiving psychotropic medications or Medication Assisted Treatment (MAT) are supported through consistent collaboration with prescribers and care teams.

We maintain regular contact with psychiatric and MAT providers to ensure medication plans are accurate, up-to-date, and aligned with each resident’s clinical progress.

Staff observe for side effects, behavioral changes, or medication challenges and provide timely updates to prescribers and case managers.
3. Support With Appointments & Refills
We assist residents with scheduling follow-up visits, coordinating transportation (if needed), and ensuring timely prescription refills.
4. Documentation & Information Sharing
Relevant information is documented clearly and shared securely with authorized providers to support integrated, whole-person care.
5. Resident Advocacy & Education
Residents are supported in understanding their treatment plans and encouraged to actively participate in discussions with their prescribers.

All care coordination efforts are embedded into each resident’s Individualized Treatment Plan, ensuring that psychiatric and MAT services are part of a cohesive recovery approach.
